Failed To Return (FTR)

If you have attended a routine check-up appointment recently and your dentist recommended a course of treatment, this will have been opened in your check-up appointment.

If you have received an E-mail or SMS to say you have FTR then please see below:

If you would like to continue with this course of treatment, please can you contact the practice as soon as possible by emailing brockstreetdentalpractice@gmail.com so that we can arrange an appointment  for you.

If we do not hear from you within 3 weeks of our communication, it will be assumed you no longer wish to go ahead with the recommended treatment and the course of treatment will be closed on the system.

Please note, if you decide to continue the treatment at a later date, a new course of treatment will have to be opened which will incur further NHS treatment charges.

You currently have no further appointments booked, if you do not attend the practice for a check-up within a 2-year period, you will be de-registered. Please see our practice policies.
